Lagos Mandates Health Insurance for All
In a landmark move, the Lagos State Government has announced the enforcement of mandatory health insurance for employers, employees, and residents, with a grace period lasting until December 31, 2026.
Governor Babajide Sanwo-Olu unveiled the initiative during a launch event at the Lagos Continental Hotel, Victoria Island, aimed at providing healthcare access for all regardless of financial capability. The program, known as the Lagos Private Health Plan (LPHP) Partnership, will officially take effect on January 1, 2027.

Sanwo-Olu emphasized that the initiative is rooted in Executive Order EO/BOS/01/2024, signed on July 16, 2024. The government aims to expand health insurance coverage to 10 million residents, significantly alleviating the financial burden associated with healthcare.
At the launch, 21 Health Maintenance Organisations (HMOs) were introduced as partners in this ambitious endeavor.
The governor highlighted the necessity of a more inclusive healthcare financing system to support both formal and informal workers, as well as vulnerable populations who previously struggled to access essential medical services.
Sanwo-Olu pointed out that the existing Ilera Eko scheme primarily focused on vulnerable groups but needed to be complemented by a structured health insurance option for salaried workers and corporate entities.
He assured that the state would implement strict compliance measures, including the establishment of a Steering Committee and an Enforcement Task Force.
“We want compliance, not unnecessary confrontation,” Sanwo-Olu stated, urging all stakeholders to utilize the transition period for proper enrollment and plan selection.
The initiative is designed to create predictable payment structures for healthcare providers, enhancing service delivery while reducing the burden on public hospitals.
The health insurance contributions will also support the Lagos State Health Equity Fund, aimed at extending coverage to residents who cannot afford insurance.
The governor assured that performance monitoring of HMOs would be a priority, focusing on timely claims settlement and access to care.
State Health Commissioner, Prof. Akin Abayomi, called the launch a pivotal shift in the healthcare financing landscape.
He emphasized that out-of-pocket payments often lead to severe financial strain and that mandatory participation will create a robust insurance pool, reducing the risk of families facing financial ruin due to illness.
The LPHP is set to increase insured residents from about two million to at least 10 million, with active support from accredited HMOs.
Special Adviser to the Governor on Health, Dr. Kemi Ogunyemi, clarified that the initiative aims to expand choices for individuals and employers while fostering a well-regulated insurance framework.
The Lagos State Health Management Agency (LASHMA) has ramped up public awareness campaigns to inform residents about the new health insurance requirements.
Funding for the Lagos State Health Equity Fund has increased significantly to enhance healthcare access for vulnerable communities.
Dr. Adebayo Adedewe, LASHMA’s Chairman, reiterated the importance of stakeholder collaboration to ensure the success of the LPHP.
The program will utilize a digital platform to streamline enrollment and employer registration, supported by technology partnerships.
Healthcare leaders have welcomed the initiative while calling for transparency in implementation and timely reimbursement for providers.
They stressed that success should not only be measured by enrollment numbers but also by residents’ ability to access necessary treatments without financial stress.
As the grace period approaches its end, the Lagos State Government is committed to ensuring that the LPHP provides robust healthcare options to all, setting a new standard for health insurance in Nigeria.
